越南帮三兄弟：大哥阿渣（吕良伟）霸气纵横、二哥TONY（邹兆龙）冷静狡猾、三弟阿虎（行宇）打遍天下。三兄弟于黑道日益壮大，要除掉死对头老鬼山（林国斌）。血战当中，眼见成功在望，却想不到身边得力保镖华生（古天乐）竟是马军安插下来的警方卧底。
马军火爆的行事手法受千夫所指，华生也将痛苦与责任推到马军身上。终于马军展开拘捕行动，就在此刻华生的卧底身份被揭穿，惨被推下车重伤昏迷。马军最后只能将阿虎截击下来。迫使阿渣与TONY败走……
　　这次败走，却没有让阿渣退却。行动变本加厉，一夜间能指证他们的证人通通被消灭，警司（郑则仕）意外被炸死，最后只剩下已成拐子的华生能做证人，但他深爱的女友（范冰冰）却被带走！新任上司（许睛）因马军违反警方条例而要将他逮捕，马军为救华生及将渣哥一党绳之于法，不顾一切与他们拼死决斗……
